Please complete this form to apply to be a vendor at our farm market on Local Vendor Saturdays which take place from 9 am to 1 pm on Saturdays from May through October

Please note:  This is not a permit form.  Applicants must pursue their own permits with the County when needed.

The following section helps Luther's Farm Market maximize product offerings at our market and allows us to monitor and audit vendors to ensure products are being produced by the vendor.  Please specify which items you wish to bring and sell at the market. 
Are the products listed above raised or produced on your farm or at your residence?
Are your products certified organic?
Do you have vendor liability insurance which covers incidents that may occur off your premises?
Do you have product liability insurance?
